01 The clinical problem

Every millimeter of bone
matters.

In the posterior maxilla, atrophy and sinus pneumatization leave only 3, 4, sometimes 2 mm of bone where 10–12 mm would be needed. Crestal sinus lift is a minimally invasive option in selected cases — but the traditional toolset works against you.

Burs, oscillating tips and ultrasonic devices remove bone where every millimeter counts. Depth and direction depend on operator feel. Irrigation floods the field. And the radiograph can still show a dome that is in fact fibrous tissue, not bone.

02 The solution

Bone preserved,
not removed.

The Magnetic Mallet delivers a 50-microsecond impulse with 5,000 g of acceleration and 1 mm of excursion. Bone is not cut: it is displaced, compacted and remodeled. The result is true osseodensification along the tunnel walls — improved bone quality and primary stability where it matters most.

With no rotary heat, no irrigation is required: the surgical field stays dry and fully visible throughout the procedure. Kit Elevate applies this principle with five concave-tipped inserts that collect autogenous bone particles and carry them apically as regenerative material.

Rotary bur-free preparation preserves the integrity of the bone matrix: osteocytes remain viable, periosteal vascularization is not compromised, and the tissue responds with measurable changes in osteogenic growth factor expression. What is Kit Elevate and what problem does it solve?

Kit Elevate is a system of five concave-tipped inserts designed for crestal sinus elevation with the Magnetic Mallet. It addresses a fundamental limitation of rotary burs: they remove the very bone clinicians aim to preserve. Kit Elevate compacts and repositions bone instead, providing millimeter-level control from 1 to 13 mm.

How does Kit Elevate help avoid Schneiderian membrane perforation?

It uses three layers of protection: depth stops that physically limit penetration to the selected working depth; a controlled fracture with Elevate 5, which creates a protective tenting effect; and, in more extreme cases, the 2STEPS protocol with interposed collagen.

When should I choose a crestal approach instead of a lateral approach?

Measure the sinus width at 10 mm on the CBCT scan. In a narrow sinus (<12 mm), the crestal approach with Kit Elevate is preferred because the graft material remains in contact with the sinus walls, supporting vascularization. In a wide sinus (>12 mm), a lateral approach is typically indicated.

How much residual bone height is needed?

Kit Elevate is used with 2 to 7 mm of residual bone height. With 5–7 mm, a simple mini sinus lift is generally sufficient. With 3–5 mm, the complete protocol can often be performed in a single session. With 2–3 mm, the 2STEPS protocol is recommended. Above 7–8 mm, sinus augmentation is usually not needed.

Is immediate loading possible?

Yes, when primary stability and the clinical conditions allow it. After placement, stability can increase spontaneously, especially during the first 15 minutes, as compressed cortical bone tends to return toward its original shape. This can support immediate loading even in low-density bone.

Is vibration or impact transmitted during the procedure?

The 50-microsecond impulse is below the threshold typically perceived as vibration. Clinical reports consistently describe little to no perceived shock. There is no continuous rotary vibration, no heat generation, no rotary handpiece noise, and no irrigation required. The post-operative course is typically reported as smoother, with less swelling and faster healing.

Is Kit Elevate compatible with Magnetic Mallet handpieces?

Yes. The five inserts fit new-generation Magnetic Mallet handpieces and use the impulse generated by MAGNETODYNAMICS technology. Power is adjustable across four levels; about 90% of procedures are performed at levels 1–2.
